Project Summary:
Opex was instructed to review the facility and audit maintenance activity, process, and procedures with respect to the ventilation failure incident. This included the reactive maintenance regime; the planned and preventative maintenance regime; the role of the Help Desk system; and the compliance with the contractual requirements in respect of the ventilation system against the contract and any statutory requirements.
Opex were brought in to provide an unbiased investigation as to what was the cause and act as an expert witness.
